The ovary and the uterus - from basic science to clinical application

Sammanfattning av projektet

Huvudsyftet med projektet är att genom experimentella, kliniska och epidemiologiska studier som berör ett antal vanliga kvinnosjukdomar öka vår kunskap för att förbättra diagnostik, behandling och omhändertagande efter behandling.
Frågeställningar:
1. Vilka LH inducerade gener är ovulations-specifika i det human ovariet och kan styrning av dess produkter användas för att stimulera ovulation vid infertilitet och inhibera i kontraceptionssyfte?
2. Vilken roll i OHSS patofysiologin har ett antal överuttryckta gener och hur kan denna kunskap användas vid prevention och behandling av OHSS?
3. Hur utvecklas PCOS hos kvinnor efter klimakteriet och finns förändrat genuttryck av patofysiologisk betydelse i PCOS ovarium?
4. Finns skillnader i hormonstatus, sexualitet och psykologiskt välbefinnande 8 år efter elektiv hysterektomi hos kvinnor som har genomgått oophorektomi i jämförelse med de som ej genomgått oophorektomi?
5. Kan steg-för-steg förfarande med djurexperiment bana vägen för klinisk introduktion av uterustransplantation?
6. Kan ovariekryopreservation optimeras för bevarande av fertilitet hos flickor och kvinnor som drabbas av cancer?
7. Hur är utfallet vid behandling av ovarialcancer i VGR och kan de kirurgiska behandlingarna utvecklas?
8. Kan ny teknik användas för att särskilja cervixcancer och dysplasi hos gravida och hur påverkas sexualitet och ovariefunktion efter kirurgisk behandling av cervixcancer.
9. Är laparoskopisk eller vaginal operation bäst vid operation av framfall efter hysterektomi?
The main goal is to improve diagnostics, treatment and postoperative care for a number of common female health issues by performing experimental, clinical and epidemiological studies.

1. Which LH-induced genes are ovulation-specific in the human ovary and can pertubations of these stimulate ovulation at anovulation or inhibit ovulation for contraception?
2. Which roles do a number of newly found genes, which are expressed in granulsa cells of OHSS patients, play in OHSS patophysiology?
3. What is the natural cause of PCOS after menopause women and is the expression pattern changed in PCOS ovary?
4. Are there differences in hormonal status, sexuality and psychological wellbeing 8 years after prophylactic oophorectomy in conjunction with hysterectomy as compared to only hysterectomy?
5. Can a step-by-step animal experimentation approach lead to clinical introduction of uterus transplantation?
6. Can ovarian cryopreservation be optimised for fertility preservation in young females with cancer?
7. How is the clinical outcome of treatment for ovarian cancer in western Sweden and can the surgery be improved?
8. Can new technology be used to discriminate dysplasia and microinvasive cervical cancer in pregnant women and how do sexuality and ovarian function change efter surgical treatment for cervical cancer?
9. Is laparoscopic or vaginal surgery best for correction of vaginal vault prolapse post hysterectomy?

Ovary
The reproductive organ (GONADS) in female animals. In vertebrates, the ovary contains two functional parts: the OVARIAN FOLLICLE for the production of female germ cells (OOGENESIS); and the endocrine cells (GRANULOSA CELLS; THECA CELLS; and LUTEAL CELLS) for the production of ESTROGENS and PROGESTERONE.
Uterus
The hollow thick-walled muscular organ in the female PELVIS. It consists of the fundus (the body) which is the site of EMBRYO IMPLANTATION and FETAL DEVELOPMENT. Beyond the isthmus at the perineal end of fundus, is CERVIX UTERI (the neck) opening into VAGINA. Beyond the isthmi at the upper abdominal end of fundus, are the FALLOPIAN TUBES.

Polycystic Ovary Syndrome
A complex disorder characterized by infertility, HIRSUTISM; OBESITY; and various menstrual disturbances such as OLIGOMENORRHEA; AMENORRHEA; ANOVULATION. Polycystic ovary syndrome is usually associated with bilateral enlarged ovaries studded with atretic follicles, not with cysts. The term, polycystic ovary, is misleading.

General term for CYSTS and cystic diseases of the OVARY.
Ovarian Hyperstimulation Syndrome
A complication of OVULATION INDUCTION in infertility treatment. It is graded by the severity of symptoms which include OVARY enlargement, multiple OVARIAN FOLLICLES; OVARIAN CYSTS; ASCITES; and generalized EDEMA. The full-blown syndrome may lead to RENAL FAILURE, respiratory distress, and even DEATH. Increased capillary permeability is caused by the vasoactive substances, such as VASCULAR ENDOTHELIAL GROWTH FACTORS, secreted by the overly-stimulated OVARIES.
Transplantation
Transference of a tissue or organ, alive or dead, within an individual, between individuals of the same species, or between individuals of different species.
Ovariectomy
The surgical removal of one or both ovaries.
Castration
Surgical removal or artificial destruction of gonads.
Endocrine Surgical Procedures
Surgery performed on any endocrine gland.
Ovulation
The discharge of an OVUM from a rupturing follicle in the OVARY.
Reproduction
The total process by which organisms produce offspring. (Stedman, 25th ed)
